Treatment Overview

Liposuction removes fat from your body using suction. During liposuction, small, thin, blunt-tipped tubes (cannula) are inserted through tiny cuts in the skin. Fat is suctioned out through these tubes as the doctor moves the tubes around under the skin to target specific fat deposits.

In recent years, improved techniques have made liposuction safer, easier, and less painful. These newer techniques include:

Tumescent liposuction. First, you are given a local anesthetic to numb the area of your body where the tube will be inserted. Next, a large amount of an anesthetic solution containing lidocaine and epinephrine is injected into the fatty tissue before traditional liposuction is performed. The solution makes the fat expand and become firmer, which allows the tubes to move more smoothly under the skin. It also causes the blood vessels to temporarily shrink (vasoconstriction), which greatly reduces blood loss during the procedure. With the large volume of anesthetic solution injected, tumescent liposuction may not require general anesthesia (which makes you sleep through the procedure) and thus does not carry some of the risks of traditional liposuction. It also reduces bruising, swelling, and pain after the procedure.
Ultrasound-assisted (ultrasonic) tumescent liposuction. This newer technique uses energy created by ultrasound to liquefy the fat before it is removed. The tubes move more smoothly under the skin after the fat has been liquefied, so this technique may be particularly helpful in areas where the fat is very firm or fibrous—such as the sides, the back, male breasts, and the area around the navel—or when removing large amounts of fat. Ultrasound-assisted liposuction may take 2 to 4 hours longer than standard tumescent liposuction.
Laser-assisted tumescent liposuction. This technique uses a laser to liquefy the fat before it is removed. Similar to ultrasound-assisted liposuction, this helps the surgeon remove the fat from under the skin. A laser also may be used to tighten the skin over the area where the fat was removed.

Liposuction is usually done as an outpatient procedure in a properly equipped doctor's office, ambulatory surgery center, or hospital. In general, it does not require an overnight hospital stay unless a large volume of fat is being removed. But liposuction of large areas of the body should only be done in a hospital or licensed surgical center.²Local anesthesia is used in some cases, and you may or may not be given a sedative to help you relax. If a large area or volume of fat is being treated, general anesthesia or deep sedation with a local anesthetic may be used.

Between 50% and 60% of the transferred fat survives. This means a certain amount of overcorrection is required which also contributes to some of the swelling following surgery.

My whole post-adolescent life

I've put a lot of effort into rectifying its disproportionately large size and as a competitive City girl have battled with a string of unbearable diets. Over the past six years I've attempted Atkins, low GI, low calorie (well, starvation really), the cabbage soup diet and even Beyoncé's maple syrup concoction.I've tried personal trainers, yoga, Pilates, power plates and Brazilian boot camps, yet I still look dreadful in jeans. So when I recently heard whispers around the City about a new form of "lunchtime lipo", my ears pricked up. Rumour had it that the newest form of fat removal caused only minimal bruising and downtime - so virtually no time off work - and was taking London by storm. After five minutes on Google I'd located Dr Ayoubi, the man credited with introducing the treatment to the UK.
Dr Ayoubi, a charming and softly spoken Syrian, first explained the philosophy of his clinic, which is that any surgery is aimed at "enhancing" rather than "altering" looks. I had been hoping for more shrinkage than "enhancement".
The treatment can be applied to any part of the body though the most popular areas are post-natal tummies, saddlebags, double chins and bingo wings. Not surprisingly, 80 per cent of Dr Ayoubi's smart lipo clients are women but he noted, "We do get men. Most of them are looking to remove 'man breasts'." I don't blame them.
The treatment is not a cure for obesity. It's designed for patients with areas of "stubborn fat".
After granting that the fat around my rear was suitably stubborn, I contemplated an appointment the following week, which would make me "bikini-ready" three weeks after that.
With a holiday to Europe looming and a beach full of sun-kissed supermodels on my mind, I handed over my credit card.
On my way to the treatment, my optimism faded and I was instead questioning my own sanity. Fortunately my fears were allayed on arrival at the fantastically chic Hurlingham Clinic (and spa, incidentally) in Parsons Green.
The atmosphere was calming - it felt like I was just swinging by for a facial - and the staff couldn't have been more friendly.
As the appointment loomed, however, the luxurious surroundings weren't enough to appease my fears - in my nervous state I embarrassingly put on my surgical gown backwards and forgot my operating knickers.
A couple of Valiums later, and with two motherly nurses holding each hand, I found myself on the operating table.
After the application of local anaesthetic, it was time for the laser treatment, an odd sensation to say the least.
Having made a very small incision, the surgeon inserted a two-milimetre-wide laser under the skin, and melted the fat cells that had been causing me so much distress.
Ice packs were applied after each zap to ensure that the skin wasn't burnt.
The first version of Smart Lipo would leave it at that, but this new generation treatment (SmartLipo MPX) uses a little suction to remove some of the fat there and then, so that a partial result may be seen immediately and the end result is more effective.
"I have found that patients are anything but patient," Dr Ayoubi joked. I knew that my surgeon and I were on the same wavelength.
When I gently sat up, trying not to look too carefully at my mangled body, I noticed the sponge-like texture of my thighs before I was quickly manoeuvered into the garment I was to wear for the next 10 days.
These were like a slightly tighter, nude pair of Bridget Jones style "magic knickers", with a strategically placed hole.
Although the bruising is dramatic, the procedure and recovery period has been uncomfortable rather than painful.
What's more concerning is that two weeks later my backside is still more Kerry Katona than the Carla Bruni bottom I had envisaged.
Reassuringly, Gillie Turner, the helpful managing director of the Hurlingham Clinic, reminded me that "the treatment was not a miracle cure, that I should continue to eat sensibly and take exercise and that the full effects would become apparent in "about three months' time".
Providing my thighs no longer "kiss" and I'm happily wearing my skinny jeans by October, that's enough of a miracle for me.

Breast cancer cases surge as cervical dips

New Delhi (India): The good news: fewer women in India now suffer from cervical cancer — counted among the worst of killers. The bad news: breast cancer cases have shot up alarmingly across the country.
A landmark analysis of cancer cases in Delhi, Mumbai, Chennai and Bangalore between 1982 and 2005 by the Indian Council of Medical Research (ICMR) has found that while cervical cancer cases dipped, in some cases by almost 50%, the incidence of breast cancer doubled. And, the trends mentioned in ICMR’s yet-to-be-released report ‘Time Trends in Cancer Incidence Rates (1982-2005)’, were universal in all four cities.
While Bangalore saw breast cancer cases more than double since 1982 — 15.8 in a population of one lakh in 1982 to 32.2 in 2005 — Chennai recorded 33.5 new cases of breast cancer in 2005 against 18.4 in 1982.
Delhi recorded 24.8 new cases of breast cancer a year per 100,000 women which rose to 32.2 in 2005. Mumbai recorded 20.8 new cases of breast cancer per 1 lakh women in 1982 which rose by almost 10% in 2005.
‘‘Factors like late age of marriage and fewer children could be responsible for the decline,” said ICMR director general and secretary of department of health research Dr V M Katoch.
Chief of medical oncology at AIIMS (All India Institute of Medical Sciences) and head of the Delhi Cancer Registry Dr Vinod Raina agreed, ‘‘Increasing number of women are now delivering in institutions which has greatly improved personal hygiene. Women now marry late and give birth to fewer children, all of which have led to a dip in cervical cancer cases.’’
Ironically, the same reasons, says Dr Raina, have led to increased breast cancer rates in India. ‘‘Western lifestyle, increased consumption of fat products, obesity, late marriages, delayed child bearing and less number of children being conceived leading to reduced breastfeeding and use of some contraceptives, are all believed to be behind this increased risk of breast cancer,” he said. “This cancer is also inevitable with an ageing population.’’
Dr Raina added that breast cancer cases in India are fewer than in the West which records around 100 new cases per 100,000 population every year. Cervical cancer: Chennai sees 50% fall in cases
New Delhi: While the number of women suffering from cervical cancer in India has come down, breast cancer cases have shot up alarmingly.
In case of cervical cancer, this is how the cases have dropped: In 1982, Bangalore reported 32.4 new cases of cervical cancer in women per 100,000 population every year. The number dipped to 27.2 in 1991, 17 in 2001 and 18.2 in 2005.
Delhi, whose records are available from 1988, saw 25.9 new cases of cervical cancer per 100,000 population the same year. It then dipped to 19.1 in 1998 and then to 18.9 in 2005. Mumbai, which recorded 17.9 new cases of cervical cancer per 100,000 population in 1982, recorded 12.7 new cases in 2005.
Chennai recorded a fall of almost 50% in cases of cervical cancer in this period of 24 years. In 1982, Chennai recorded 41 cases per 100,000 population; nearly a decade later, in 1991, Chennai’s figure of new cases dipped to 33.4. In 2005, new cases fell further to 22 per 100,000 population. Caused by the Human Papiloma Virus, cervical cancer is often called the poor woman’s disease.

A simple procedure used in cosmetic surgery could cure migraines permanently, American surgeons have claimed.

The operation, similar to that used when carrying out a forehead lift, has been found to “permanently fix” the debilitating effects of a migraine, which include crippling headaches, dizziness, sensitivity to light and nausea, the Scotsman reports.

So far, more than 40 of the ground-breaking procedures have taken place and the surgeon in the United States who pioneered the technique says more than half the patients reported “complete elimination” of migraine symptoms.

For nearly a decade, researchers have been testing the concept that migraines are caused when a person’s “trigeminal nerve branches”, which are located on the side of the skull, are irritated.

When the muscles around these branches are incapacitated, the headaches stop, which is why some patients have found relief from the “freezing” effect of Botox treatments. According to the study, removal of these muscles or “triggers”, offers an easily attainable and permanent fix.

And experts say not only does the procedure have a “colossal impact” on a patient’s quality of life, it can also “eliminate the signs of aging” at the same time.

The condition was recently named as the top reason given by people when phoning in sick to their work and severe migraine attacks are classified by the World Health Organisation as among the most disabling illnesses.

Study author Bahman Guyuron, from the department of plastic surgery at University Hospitals Case Medical Centre, said: “In this study, we’ve shown surgical treatment of migraine headaches is safe, effective, and that this reasonably short operation can have a colossal impact on the patients’ quality of life — all while eliminating signs of aging for some patients, too.”

Guyuron added: “Though one might not think to look to plastic surgeons to treat migraines, we are commonly involved in peripheral nerve surgery and treat nerve-related pain, so this is a meaningful addition to the field of reconstructive plastic surgery.”

Surgeon carries out amputation by SMS

A British surgeon volunteering in the Democratic Republic of Congo saved the life of a teenage boy by amputating his shoulder using instructions SMSed by a colleague in London.
Vascular surgeon David Nott helped the 16-year-old while working 24-hour shifts with medical charity Medecins Sans Frontieres (MSF) in Rutshuru.
The boy’s left arm had been ripped off and was badly infected and gangrenous, reports BBC News website.
Nott, 52, from London, had never performed the operation but followed instructions from a colleague who had.
The surgeon, who is based at Charing Cross Hospital in west London, said: “He was dying. He had about two or three days to live when I saw him.”
Nott said he had been told the boy had been bitten by a hippopotamus while fishing, although he later discovered he had been caught in crossfire between government and rebel forces.
There were just 6 inches of the boy’s arm remaining, much of the surrounding muscle had died and there was little skin to fold over the wound.
Nott knew he needed to perform a forequarter amputation, requiring removal of the collar bone and shoulder blade. He contacted professor Meirion Thomas, from London’s Royal Marsden Hospital, who had performed the operation before. “I texted him and he texted back step by step instructions on how to do it,” he said. “Even then I had to think long and hard about whether it was right to leave a young boy with only one arm in the middle of this fighting. “But in the end he would have died without it so I took a deep breath and followed the instructions to the letter. I knew exactly what my colleague meant because we have operated together many times.”
The operation is only performed about 10 times a year in the UK, usually on cancer patients, and requires the back-up of an intensive-care unit. Patients usually lose a lot of blood during the procedure. Nott, from Fulham, west London, had just one pint of blood and an elementary operating theatre, but the operation, performed in October, was a success and the teenager made a full recovery. The surgeon, who volunteers with MSF for a month every year, said: “I don’t think there’s more than two or three surgeons in the UK who can do this. It was just luck that I was there and could do it. “I don’t think that someone that wasn’t a vascular surgeon would have been able to deal with the large blood vessels involved. That is why I volunteer myself so often, I love being able to save someone’s life.” In the absence of intensivecare facilities, Nott said he had personally monitored the boy’s recovery from his bedside, tending his wounds.

Quarter of men think wife needs boob job

A poll of 2,000 married men revealed that 16 per cent think their other half needs plastic surgery and six per cent said their wife might benefit from "a complete overhaul".
Almost a third said it would spice up their sex life and 19 per cent reckoned plastic surgery could save their marriage.
Nearly a quarter of men (23 per cent) said they missed the body their wife had when they first met.
Although 81 per cent were in agreement that a woman's body is under more pressure to endure more than a man's and 41 per cent of blokes said having children had taken its toll on their wives' figure.
One in 10 said their wife could do with some botox, according to the poll.
The most popular procedure men wanted their wife to have was liposuction on their tummy, followed by lipo on their legs, with breast enlargement third most popular.
Over 35 per cent of men said they would never tell their wife she needed plastic surgery as she would be too offended but 17 per cent said they would approach her because she might be open to their suggestions.
Over a third of blokes were so keen for their wife to go under the knife, they were prepared to pay for it but a quarter said they believed their wives would like them to have cosmetic surgery.
Three quarters of chaps said they would never go under the knife to save their marriage or to spice up their sex life.
The survey was commissioned by Opera North, ahead of the world premiere of Armando Iannucci's operetta, Skin Deep.
The work deals with a society preoccupied with celebrity and ordinariness and opens at Leeds Grand Theatre on January 16 and will tour to London, Manchester, Newcastle and Bregenz, in Austria.
A spokesman said: "It seems that a lot of men think their wives could do with surgery to enhance their looks but looking young and gorgeous isn't the be all and end all to a marriage.
"Plastic surgery is so much more on people's radars and the survey shows a lot of men do actually see it as a viable option to spruce up their wives. It's particularly amusing that very few men would be prepared to do the same thing."

Kerry Katona left with wrinkled belly after plastic surgery

But it seems ex-jungle queen Kerry Katona might need a return visit to the surgeon to, erm, iron out a few wrinkles. The troubled mum-of-four, 28, stepped out yesterday looking like crepe - as her fitted sweater rode up to reveal a crinkly tummy and loose skin peeking over the top of her favourite skinny jeans. Kerry, who was declared bankrupt in the summer and lost a lucrative magazine contract after her shambolic appearance on This Morning, has undergone extensive cosmetic surgery this year.
It included liposuction to her jelly belly, breast reduction from 34GG to 34DD and the removal of stretch marks. But Kerry, who recently admitted she is an alcoholic, got some puzzled glances from onlookers yesterday as she strolled near her home in Wilmslow, Cheshire.
One said: "She looks great - she's slim and is obviously proud of her new figure. But it's not quite as perfect as she'd have us believe.
"Like most mums, she's never going to have a washboard tummy. And having all that excess fat removed is bound to have left her with some wrinkles."

Shock cost of W Yorks cosmetic surgery ops

NEW figures today exposed the shock cost to West Yorkshire's NHS of the growing demand for cosmetic surgery.
The YEP can reveal that hospitals in Leeds, Wakefield and Kirklees carried out more than 650 so-called 'non-medical procedures' in the past two years – at a cost to the taxpayer of hundreds of thousands of pounds.

News of the expenditure comes at a time when there are 150 people waiting for hip replacements at Leeds hospitals and another 100 people in the Wakefield and Dewsbury area. A typical hip op costs around £7,500.
Now, campaigners at the Taxpayers' Alliance said the NHS needed to get its priorities right.

Alliance director Mark Wallace said: "It is a cause for concern when people are getting free cosmetic surgery whilst other patients cannot get treatment for life-threatening conditions."
Local NHS managers say each was considered individually and weighed up against national criteria for "restricted treatments".
They stress, however, that the ops are never carried out for solely cosmetic purposes and that many of the people treated would have been suffering from psychological problems.
But Mr Wallace added: "This is a question of having the right sort of priorities. Someone with low self esteem can be helped without necessarily having invasive, dangerous, expensive surgery, whereas someone whose heart doesn't work properly has no choice.
"People with a clear medical need have to be the priority for the NHS."
In Leeds, bosses shelled out for 476 procedures between June 2006 and June 2008.

In Wakefield, there were 105 non-medical treatments, including: 20 nose jobs, 14 tummy tucks, nine procedures to remove male breasts, nine ops on droopy eyelids, seven to correct breast asymmetry and one to separate toes.
Bosses at Kirklees NHS approved 76 treatments including plastic surgery for stretchmarks, laser hair removal, breast enlargement, liposuction and to fit gastric bands.

The figures were obtained by the YEP using the Freedom of Information Act.

According to NHS averages, a tummy tuck costs around £4,900, the procedure to remove male breasts (gynaecomastia) around £2,300, pinning back ears costs about £1,100 and a nose job around £1,300.
A spokeswoman for Leeds NHS said they considered each case carefully.
She added: "Applications for procedures termed as cosmetic are made on behalf of patients by their GP after careful consideration of the potential physical and emotional impact."
Wakefield District NHs senior commissioning manager Shaun Boffey said: "NHS Wakefield does not fund any purely cosmetic surgery. We base our decision to approve treatment on the evidence of clinical need."
A spokeswoman for Kirklees NHS said: "We do not fund procedures for cosmetic or aesthetic reasons and we follow strict criteria before restricted treatments are provided. Each case is assessed individually and needs to have exceptional reasons.
